Poverty line Scheme available at Mahatma Gandhi Hospital,New Pargaon:-

1. Below Poverty Line (BPL) Health Care Scheme: - (Basic Terms and conditions apply)

(For all concerned within this Region / District.)

         2. Above Poverty Line (APL) Health Care Scheme: - (Basic Terms and conditions apply)

(For all concerned within this Region / District.)

Details:-

As per mandatory directions of Hon’ble High Court with respect to functioning of Public Charitable Medical Trusts:-

Below Poverty Line (BPL) Health Care Scheme: -                                                                   

- For all concerned within this Region / District whose Annual Income is less than           Rs. 25000/- (Rupees Twenty five thousand only)

-       Are eligible to avail medical examination and treatment totally free of cost.

-       Should submit following documents to benefit under the above scheme,

i.            Annual Income Certificate from Tahsildar.

ii.            Below poverty line official card.

Above Poverty Line (APL) Health Care Scheme: -

-       For all concerned within this Region / District whose Annual Income is upto                  Rs. 50000/- (Rupees Fifty Thousand only).

-       Are eligible to avail Medical examination and treatment at Concessional rate. 

-       Should submit following documents to benefit under the above scheme,

i.              Annual Income Certificate from Tahsildar.

ii.            Below poverty line official card.

Please Note:

For availing the above facilities the following documents will be required along with the concerned beneficiary,

1. Each Member will have to produce their Membership card along with Photo Identity proof for availing the above services.
2. Each Worker will have to produce their Employment Identity Card along with Photo Identity proof for availing the above services.
3. Family members of Members will have to produce Xerox attested copies of Membership Card, Ration Card & Photo Identity proof for availing the above services.
4. Family members of Workers will have to produce Xerox attested copies of Employment Identity Card, Ration Card and Photo Identity proof for availing above services
5. Any other terms and conditions laid down for the welfare & legality of the schemes hereafter by the Management will be mandatory.
